You have to install the ContactForm7 free plugin to add the contact form.
Go to Appearance > Plugins > ContactForm7 and install it.

I created here a contact form https://www.afterlife.co.za/contact/. It seems fine.
Are you talking about images on home page? You have added then in a text box, try to use media element, or gallery.
