Please can you tell me if it is possible for a portfolio item on the homepage to link to a Lightbox image?

I would like it so that the portfolio item displays a featured image on the home page. When this is clicked, a larger image (a different file to the featured image) is displayed in a Lightbox.

Is this possible?
