  Please make sure to have installed “Contact Form 7” plugin first. I can not login to your site (screenshot).

  Hello,
  
  I see you have installed the plugin. Now all you had to do is to go to the contact form element, select the already created form and save changes. I did it for you just now.
  Please check your page. the contact form is displaying as it should. (Screenshot)
