You can add the sign up form as a widget. Go to Appearance > Widgets. Add the Mailchimp widget in the page’s sidebar (Drag & Drop).

When I follow your instructions/screenshot I don’t end up with the same information being displayed. Under the sidebar options there are two boxes that say text and nothing else.
Your example shows the navigation menu mailchimp sign up form as options from a menu.
I don’t understand. There is not a menu drop-down in the screenshot. This is only the Mailchimp widget. If you see a Mailchimp widget in your Widgets area, just drag-and-drop it in the sidebar you want it to be.
